Enable Mod_speling

I have it working for the default hosts... Check your httpd.conf, site configuration as well as .htaccess files for mod_rewrite rules which might be matching the urls you are trying to make case-insensitive. I've only ever seen or heard of issues disabling Apache's case-sensitivity.
